To expedite handling of your request for veterinary supplies or services please complete
the form below. You will receive acknowledgment of your request within 1 business
day. Supply requests fulfilled by DLAS are subject to a 20% service fee that will
be included in the total cost.
- If you are requesting a veterinary technical service, the service must be scheduled 8 days in advance.
- If you are submitting a request for transport boxes please include:
- Number of Boxes Needed
- Box delivery Location
- Number of dividers (maximum of 3/box)
